Insurance

I am currently accepting Aetna, UnitedHealthcare, and NYSHIP / Empire Plan. Please check the details of your plan for any co-pays, co-insurances, or deductibles that may apply. 

Self-Pay (Out of Network)

If I do not accept your insurance, I will work with you to come to an agreement on a fee that is manageable for you that is determined by a number of factors we can discuss upon meeting. If your insurance plan offers 
"out-of-network" benefits, I will assist in submitting the forms required for reimbursement.
